What is an ACO?
An Accountable Care Organization (ACO) is a group of doctors, hospitals, and other healthcare providers who voluntarily work together to provide coordinated, high-quality care to Medicare beneficiaries. The goal of an ACO is to ensure that patients—especially those with chronic conditions—receive the right care at the right time, while avoiding unnecessary services and preventing medical errors.

Dr. Paley is a proud member of the Rio Grande ValleyAccountable Care Organization (RGVACO). As part of this network, our practice, PaleyMD, is committed to providing highly coordinated andpatient-centered care.
After-Hours Access
We offer a dedicated cell phone line for minor emergencies during evenings and weekends—answered personally by Dr. Paley. As part of this coordinated approach, we offer a dedicated support phone line. This service gives you access to healthcare guidance outside of normal office hours—helping to reduce unnecessary emergency room visits and long wait times. It’s one more way we’re working to make your care more convenient and effective.
Hospital Coordination
If hospitalization is necessary, Dr. Paley works directly with our dedicated hospitalist at Methodist Stone Oak Hospital to ensure smooth admissions and continuity of care throughout your stay.
Coordinated, Personalized Care
At PaleyMD, we collaborate closely with both you and your specialists to create a customized care plan that meets your specific needs. To help us provide the best care possible, we ask that you keep us informed of the following:
- Any changes in your medications
- New specialists you are seeing
- Changes to your health status or medical conditions
Keeping your care team informed helps us make the mosteffective and informed decisions on your behalf.
Required Medicare Visits
To meet the standards of care under Medicare and the ACOmodel, we require two key visits each year:
- Medicare Annual Wellness Visit – Focused on prevention and long-term planning
- Medicare Quality of Care Visit – Designed to address ongoing conditions and ensure quality outcomes
We also ask that you schedule a follow-up visit immediately after any hospital discharge, so we can reassess your needs and ensure your recovery is on track.
